Please bring your insurance card, including any supplemental insurance, and present this at your time of check-in. Any co-payment required by your insurance must be made at the time of your service as you check out. Failure to pay your co-pay at the time of service will result in a $15.00 billing fee.

If you have a financial need, please discuss this with our financial office, and a payment schedule can be arranged. Do not hesitate to call the finance department if you need assistance or have questions.
